Hale Kōkua, care home placement

For families in Hawaii and on the mainland

Find a licensed care home in Hawaii for your parent

Tell us about your person. We look at licensed homes that may fit and introduce your family to them. After that, the home takes it from there. Families pay nothing to search.

Four steps

How a family search works

  1. 1

You tell us about your person

Use the form or email us. Tell us who your person is, the island, and what a normal day needs.

  2. 2

    We look at licensed homes

We explain which Hawaii license fits the care you describe. Diet, wheelchair, memory, money, and where family lives help us narrow the list.

  3. 3

    We introduce you to homes

With your OK, we introduce your family to one or more licensed homes that may fit, and share what you told us.

  4. 4

The home takes it from there

You talk with the home, visit, and decide. The home handles admission and care. Our part ends at the introduction.

There is no cost to your family at any step, and no pressure to choose a home.

What we ask on the first call

It is a conversation, not a form. Share what you know. We fill in the rest together.

Your person

Name, age, the island they should be near, and who will visit.

A normal day

Bathing, walking, meals, memory, night wandering, and medicines.

What matters at home

Wheelchair, special diet, a shared room, language, church, and pets.

How care will be paid

Private pay, long-term care insurance, VA benefits, or Medicaid. We tell you early which homes that opens.

Then you visit the home

A vacancy on a list is a starting point. The State report shows capacity, diet, wheelchair approval, and whether a resident needs to walk on their own.

After we introduce you, you set up a visit with the home directly. You meet the operator in person, in the house. The home tells you whether it can meet your person's care. If it is not the right home, tell us and we can introduce you to another.

What to expect on a visit

The kinds of homes we look at

Most private-pay searches land in an adult residential care home or an assisted living facility. When the care is heavier, we look at an expanded ARCH.

When Medicaid is part of the plan, the fit is often a community care foster family home. The State decides eligibility.

What each kind of home is like

Who pays for our help

Families pay nothing to search or to be introduced to a home.

When a placement happens, the care home pays Hale Kōkua a referral fee.

What you can count on

  • Families pay nothing to search

There is no cost to your family to search or to be introduced to a home. When a placement happens, the care home pays us a referral fee.

  • We introduce, then the home takes over

We introduce your family to licensed homes that may fit. After that, you work with the home directly on visits, admission, and care.

  • Every island

Oʻahu, Hawaiʻi Island, Maui, Kauaʻi, Molokaʻi, and Lānaʻi.

  • Licensed homes only

Every home on our list comes from State of Hawaii licensing records.

  • Our own list of homes

We keep an up-to-date list of licensed care homes on every island. It helps us see which homes may fit the care you describe.

  • A person reads your note

Not a call center. A person reads what you send and gets back to you by phone or email.

What Hale Kōkua does, and what it does not

Our part is the introduction. After your family is introduced to a care home, the home takes it from there.

What we do

  • Listen to what your person needs and explain the Hawaii license names in plain words.
  • Look at our list of licensed homes and ask homes about openings.
  • Introduce your family to licensed homes that may fit.

What we do not do

  • We are not a case management agency, and we do not manage, coordinate, or monitor care.
  • We do not meet families in person or go on visits.
  • We do not decide whether a home can meet your person's care. The home and your person's doctor do.
  • We are not involved after the introduction. From then on, your family works directly with the home.

Families pay nothing. When a placement happens, the care home pays Hale Kōkua a referral fee.

Questions from families

See all questions
What does Hale Kōkua do?

We introduce your family to licensed care homes in Hawaii that may fit your person. That is our whole service. Once you are introduced, the care home takes it from there: visits, questions about care and cost, admission, and the care itself.

What does Hale Kōkua not do?

We are not a case management agency. We do not manage, coordinate, or monitor care, and we do not check in on your person after they move. We do not meet families in person or go on visits. After the introduction, your family works directly with the care home. Questions about care go to the home and your person's doctor. For Medicaid community care, the case manager comes from a case management agency, not from Hale Kōkua.

Do families pay Hale Kōkua?

No. Families pay nothing to search, and nothing to be introduced to a home. When a family we introduced moves into a home, that care home pays Hale Kōkua a referral fee. The monthly rate is between your family and the home.

Do we have to choose a home you introduce us to?

No. There is no cost to your family and no pressure. Take the time you need. If none of the homes fit, tell us. We can introduce you to others, or tell you plainly what is possible right now.

How do you find homes?

We keep our own up-to-date list of licensed care homes across the islands, built from State of Hawaii licensing records. It helps us see which homes may fit the care you describe. We can ask a home about openings, then introduce your family. The home tells you about cost and whether it can meet your person's care.

Can you help when memory loss is part of the picture?

Yes. Memory care is not one of the Hawaii license names on this site. Tell us about the memory loss, including night wandering, and we introduce you to licensed homes that say they care for it. Ask each home how they do it. The home and your person's doctor decide whether it is the right level of care.

How do I know a home is licensed?

Ask to see the license, then confirm it with the State Licensing Section at (808) 692-7400. The Department also publishes vacancy reports and facility lists. Hale Kōkua does not issue licenses or inspect homes.

Tell us about your person

This is a hard decision, and you do not have to start alone. Tell us about your mom or dad. We introduce your family to licensed care homes that may fit.

Families pay nothing for the search.
