Expanded ARCH care homes in Hawaii

An expanded ARCH is an adult residential care home that is also licensed to care for people who need a nursing-home level of care.

It lets some people get that level of care in a small home instead of a nursing facility.

Also called: expanded adult residential care home, E-ARCH, nursing home level care in a home.

What it is

An expanded ARCH may admit people when a physician or APRN has certified a nursing-home level of care.

A registered nurse helps train caregivers and watches the daily care. Caregivers document 12 hours of continuing education a year.

A Type I expanded home generally has no more than two residents at that level. A Type II expanded home generally keeps it to 20 percent of its beds. The Department can allow more.

Who it fits

  • People whose doctor says they need nursing-home level care, and whose family wants a small home.
  • People already living in an ARCH whose care has grown. The Department can allow them to stay when it is in their interest.

Paying for it

Most families pay privately or with long-term care insurance.

Ask early about cost. Heavier care can change the monthly rate.

Questions about expanded arch

What is an expanded ARCH?

An expanded ARCH is licensed to admit people who need a nursing-home level of care. A registered nurse is part of the care. A Type I expanded home is generally limited to two residents at that level, and a Type II home to 20 percent of its beds.

Can my parent stay in an ARCH if they need more care later?

Sometimes. If the home has an expanded license, the Department can allow a resident to stay when nursing-home level care begins, after talking with the resident, family, physician, case manager, caregiver, and operator.

Where this list comes from

These homes come from public records of the Hawaii Department of Health, Office of Health Care Assurance (OHCA). The full list of licensed ARCH, expanded ARCH, assisted living, and community care foster family homes comes from the OHCA facilities roster that the State publishes as open data. We check it every day and update this list when it changes.

The monthly ARCH vacancy report only covers homes that reported to the State that month, and openings change quickly. We do not post openings. We can ask a home about openings when we introduce you.

Hale Kōkua does not own, run, or license these homes. Foster family homes are private family homes, so we list them by town and license number only, never by the caregiver's name. Confirm any license with the State Licensing Section at (808) 692-7400. Last checked October 22, 2026.
